Slide Issuance and Lending Procedure

Since pathologic records and pathology slides are important components of patient’s medical records, they should not be exposed to third parties, so these medical records require identifying the borrower and must go through a reading and copying process.

Slide Issuance and Lending Procedure
Reception of recurrent treatment : Each department for outpatients and wards (preparing an application for issuing a copy of medical records)
Visiting the reception window of the corresponding department
Issuing a copy of medical records : Windows No. 31~33 on the 2nd floor of the main building (including a copy of the pathology examination medical record)
Submission : Submit copy of medical records to the recording room of Pathology Department on the 3rd floor of the main building
Comparison with the original : Compare copy of the pathology examination records with the pathology slides (it takes about 10 ~ 20 minutes.).
  1. Histopathology slides
  2. Slide production prescription of the Pathology Department (based on the corresponding year’s insurance fee)
  3. Payment of the slide production cost (main building 2F)
  4. Receiving the slides (receiving slides after presenting the receipt to the recording room of the Pathology Department on the 3rd floor of the main building after 1 hour 30 minutes)

Submission : Submit copy of medical records to the recording room of Pathology Department on the 3rd floor of the main building
Comparison with the original : Compare copy of the pathology examination records with the pathology slides (it takes about 10 ~ 20 minutes.).
Prescription : Slide production prescription (based on the corresponding year’s insurance fee)
Payment : Payment at the 2nd floor of the main building
Receiving slides : Receiving slides after presenting the receipt to the recording room of the Pathology Department on the 3rd floor of the main building after 1 hour 30 minutes

Documents Required

  • The patient’s medical records shall not be accessed by an individual other than the patient himself/herself, neither to view or copy the record in accordance with Article 21 (2) of the Medical Law, but it is possible to issue a copy only if it satisfies Article 21 (3) of the Medical Law, and Article 13 (3) (requirements such as reading records) of the Enforcement Regulations of the Medical Law.
  • A consent form and power of attorney must be prepared and signed by the patient himself/herself. (Thumbprint or a seal is not accepted)
  • Minor: Under 14 years old
  • When an individual older than 14 years delegates consent to access his/her records, a consent form and power of attorney shall be prepared.
Applicant Required documents
Patient
  • ID card of the patient (resident registration card, driver’s license, etc.)
  • Minor (less than 14 years old): A copy of the ID card of the legal representative, family relationship certificate
Immediate family
  • Consent form (signed by the patient), a copy of ID cards (patient, applicant), family relationship certificate
  • When the patient’s age is between 14 ~ 17 years old: Patient's consent form is required.
Representative
  • Power of attorney and consent form (signed by the patient), a copy of identification cards (patient, applicant)

Notices

  • Paraffin block cannot be lent.
  • Even if you have already received a copy of your medical records and submitted it to another hospital, if you need to borrow pathologic slides, please reissue a copy of the pathology examination medical records or attach a copy of the pathology examination medical records you submitted so that you can borrow pathologic slides.
  • Histopathology slides: Reproduce and issue it after reproduction prescription of the Pathology Department and the payment of production costs (based on the corresponding year’s insurance fee)
  • After the slide is returned, the deposit amount will be refunded.
