Notice
- The patient must sign the informed consent form and power of attorney in his/her own handwriting. (cannot be replaced with a thumbprint or seal)
- Minor: less than 14 years old
- Any patient who is 14 years old or more should prepare a consent form and power of attorney when assigning the patient’s rights.
- In case of a medical certificate for military affairs, it can be issued only in person. (ID card and two 3X4 photos are required)
- Certificate retention period is 3 years.
- The medical treatment fee for the issuance of a certificate must be paid separately.
- An inpatient can receive a certificate at the 2nd floor certificate window (No. 31 ~ 33) after applying for it to the nurse's office during the hospitalization period,
- The reissuance of a certificate can be made at the certificate window without reception and medical treatment procedure.
